We do not have such QR code integration available at the moment. However, as an alternative, you may try using other tools that can generate a QR code based on the form link.
As for prefilling the form with predefined values, you can create a customized URL for your form.

Thanks, is there a way to put a “nice name” for the URL? Because if we put in the parameters in the URL, it will look really long and ugly. Is there a way to mask the URL name or put a nice name, so that in the URL it will just be a short URL as opposed to long URL containing the parameters.

That’s a valid question, thank you.
You can use an external tool to shorten the URL, as we do not offer such an option.
However, because it is just a mask, when the link is accessed, in your browser’s tab, there will still be the form’s link opened with all its parameters.
If, however, you are using the form on your website, the form’s embed code can be tweaked in order to be prefilled with the desired values.
There’s also the possibility of connecting the form with your own database, in which case you can use the form’s link without having all the values in the URL.
Database Manager is a feature we have available from our Enterprise plan.

Thanks, and is there a way for us to embed a payment gateway information such as Merchant Warrior so that our patients can make payment as they go through the form?

Hi Reza,
So you can receive payments through the form, you can integrate it with one of the payment processors we have available.
We don’t have Merchant Warrior available at the moment, however, we offer several payment gateways for integration.
Once you have added a payment processor and set up payments, the user will be redirected to a payment page after he submits the form, where he can enter his payment details and proceed with the payment.

We want to have a function where when a QR code is scanned, it will go to a 123form and prefilled with parameters
