Depending on which "Call Type" is chosen changes the pdf send to the user.

After reviewing your form, I observed that it’s currently using the older version of the Notifications. 
 
Unfortunately, the older version doesn’t support the functionality you’re looking for.
 
However, we have a workaround. By duplicating your form, it will automatically be upgraded to the new version, which enables you to achieve your desired outcome. 
 
After duplicating the form, please follow these steps to reach the final result.
 
You will have to create different notifications for each PDF (maximum of 10 notifications). 
 
Go to Set up -> New Email -> Add attachment -> Submission PDF -> Customize it as needed -> Save. Do the same for each new email template. 
 
 

 
Afterwards, you will need to create a notification rule for each template in order to send it based on the selected choice.
